인공호흡기 부조화(Ventilator dyssynchrony) - (1)

환자와 인공호흡기 간 호흡이 적절히 동기화되지 않는 상태를 의미하며, patient-ventilator asynchrony, ventilator fighting 등으로도 표현한다. trigger / cycle(terminate) / flow / other로 분류할 수 있다. 일반적으로 사용되는 pressure control mode를 중심으로 기술하였다.

 

 

Trigger dyssynchrony

Reverse trigger  (= early trigger)

machine inspiration 중 delayed된 patient inspiration이 시작되는 경우이다. reverse trigger에서 중요한 점은 호흡이 patient inspiration으로 시작되지 않고 항상 machine inspiration으로 시작된다는 점이다. pressure 그래프 상 inspiration 구간 중간에 M shape가 형성되면서 patient effort가 관찰되는 점은 double trigger 또는 air hunger와 비슷한 양상이다. 그러나 reverse trigger는 주로 deep sedation 환자에서 machine inspiration에 의한 횡격막 수축으로 뒤늦게 self가 발생하면서 나타나는 것이기 때문에, 반드시 호흡이  machine inspiration으로 시작된다는 점에서 구별된다. intervention으로는 sedative를 감량해 보거나 PS mode로 변경해볼 수 있다.

 

 

Ineffective trigger (= failed trigger)

self drive가 발생해서 pressure 그래프가 아래로 파이고 flow 그래프 상승이 관찰되지만, 둔감한 trigger로 인해 inspiration이 시작되지 않는 경우이다. pressure 그래프는 잘 나타나지 않을 수 있으므로 flow 그래프를 보는 것이 더 중요하다. 지속될 경우 fatigue를 유발하므로 trigger를 조금 더 민감하게 변경할 수 있다(weaning 단계라면 drive를 조금 더 키우기 위해 above PEEP을 약간 감량해볼 수도 있다). 만약 weaning 단계가 아니라면 sedative를 증량하거나, self drive가 지속적으로 발생하는 원인에 대해 감별하여 중재한다. 위 그래프는 inspiration이 끝나는 구간 ~ expiration이 시작되는 구간에서 지속적으로 self drive가 발생하므로 air hunger에 대한 intervention을 해볼 수 있다(early cycle일 수도 있으나 그래프 상 I:E ratio가 이미 1:1에 가까우므로).

 

ineffective trigger가 지속됨에 따라 위와 같이 air trapping, auto PEEP이 발생할 수 있다.

* self drive에도 inspiration이 아예 시작되지 않는 정도는 아니나, 약간 늦게 (100msec 이상) inspiration이 시작되는 경우를 late trigger(delayed trigger)라고 한다. ineffective trigger의 일종으로 보아도 될 것 같다.

 

 

Auto trigger (= false trigger)

trigger가 너무 민감하게 설정돼서 cardiac oscillation, ventilator circuit에 고인 응축수, 과다한 secretion 등으로 인해 triggering이 되는 경우이다. pressure가 아래로 파이는 구간이 관찰되지 않은 상태로 inspiration이 시작된다. 그 빈도가 잦으면 auto PEEP을 유발할 수 있다. intervention으로 pressure trigger와 같이 둔감하게 변경한다. 

 

auto-triggering with air leak

pressure trigger로 설정된 경우 air leak으로 인해 pressure가 감소하면서 triggering이 될 수도 있다.
